P-403 Rectal cancer in young patients – single center experience

Colorectal cancer (CRC) is the third most common worldwide and second cause of cancer-related death. The incidence early-onset CRC in those younger than 50 years rising globally. In this group, absence high-risk features screening not standard care. There strong evidence that development related to diet lifestyle. Retrospective, unicentric, cohort analysis patients with rectal diagnosed between January 2018 August 2022 ≤ old at diagnosis. Data was collected from electronic medical records regarding clinical demographic parameters (including weight), preoperative assessment, treatment, tumor response pattern disease recurrence. Obesity overweight were determined body mass index (BMI) tables. 27 meeting inclusion criteria. Median age 44 years, 14 (52%) female. Clinical staging diagnosis was: II (4%), III 20 (74%) IV 6 Curative treatment done 23 (74%). Neoadjuvant delivered 21 (91%): mostly chemoradiotherapy based on capecitabine 17 (81%). completed chemotherapy oxaliplatin (CAPOX) regimen. 1/23 total mesorectal excision complete MRI considered for watch wait strategy. curative setting, six pathologic response. majority (59%), 41% neural invasion. (27%) follow up 26,7 months, recurrence rate 22,2%, median 15,8 months our experience, young-onset frequently presents locally advanced and/or metastatic disease. High BMI seems have high prevalence, showing possible impact microbiome inflammation pathogenesis (although can’t be demonstrated conclusively study). Due small sample size, we aim perform multicentric analysis, order assess factors perhaps reappraise current strategies country.
